General Forms for Ascent Camps
REQUIRED FORMS:
Day Camps--Registration for day camps requires completion of the Permission and Medical Consent form only.
Overnight Camps--Registration for overnight camps requires completion of the Permission and Medical Consent form and the Physical Form.
If you register online, the appropriate forms will be included in your confirmation email. General information sheets for each camp can be found on the individual camp pages.
